The page jumps when you switch tabs
A live triage session on an internal engineering platform. The caller is driving the interface and describing problems as he hits them; two developers are listening and changing the product during the session.

Four things worth noticing. The clarify was not a claim — Developer A asked a question without taking the task, so Developer B was still free to own it. The claim carried a restatement, which is where a mishearing would have surfaced. The augmentation was answered aloud rather than silently absorbed, so Developer B knew the flight deck had been covered. And the result stayed labelled preliminary until the caller exercised it himself: this profile forbids closing Accepted on a change the caller has not tried.
